• A list of current prescription and over-the-counter medications you are taking. Please include your dose and frequency
• Your insurance card
• Physician referral form if required by your insurance company
• Any pertinent medical records, diagnostic testing, and blood work from last three months
• Registration and health history forms will be completed at your first visit. Please arrive 15 minutes ahead of your appointment to allow time for completion.

Our physicians are providers for Medicare and most major insurance plans. Please check your insurance company's provider listing prior to scheduling an appointment. Anything not covered by insurance will be the responsibility of the patient or guardian. Our office provides insurance billing.

Payment is requested at the time of your appointment for services not covered by insurance. Your insurance company may also require a co-payment at the time of your appointment. We accept Visa, MasterCard, checks and cash.

When necessary, our staff will work closely with patients who require a payment plan. Questions regarding your coverage and benefits should be directed to your employer or insurance company.
